Monitor
  • Renal function — most are renally cleared and renally dosed
  • Missed-dose window matters: short half-life means protection fades fast
  • Reversal: andexanet alfa (Xa inhibitors), idarucizumab (dabigatran)
🎓 Patient teaching

Do not skip doses — protection wears off within a day. Don't stop before procedures without instruction. Report falls (bleeding risk), dark stools.
